Damning verdict of official inquiry into pensioner’s killing

 

THE release from a secure hospital of a schizophrenic who killed a pensioner in the street was “indefensible”, a report said yesterday.

A damning report from Health Inspectorate Wales found multi-agency failings in the care of Martin Davies, who stabbed Gwen Poole with a bread knife as she waited for a lift to a skittles game.

HIW said it had “serious concerns” about Davies’ case, criticising Caerphilly social services for their failures and the Cygnet secure hospital in Weston-super-Mare.

It said he should never have been released in the first place and a risk assessment prepared before his release said he was “at significant risk of violence towards others”.
